Proper golf attire and shoes are required on the golf course, putting greens and practice tee. No denim or tanks tops are permitted. Proper attire for men includes golf slacks, knickers or Bermuda shorts, golf shirt with collar and golf shoes. For women, appropriate length skirts, shorts or tailored slacks with golfing blouse, shirt or sweater, and golf shoes.
For the health and safety of our guests and team members, we will follow all CDC and local health and safety guidelines in effect at the time of the event. Wearing a mask is based on your personal preference, however, if you’re unvaccinated or immunocompromised, a mask is recommended. If you are not feeling well at the time of the event, please do not attend. If CDC regulations change our event plans, we will communicate that to our guests.
*Due to production deadlines, sponsorship and logo, in .eps format, must be submitted no later than May 27, 2022, for the keepsake pin flag, and July 8, 2022 to be included on the program card.
All individuals who make a donation of $1,000 and above will become members of Advocate Health Care’s Visionaries in Health.
